Supported Community Living Offers individualized support in shared homes or through flexible hourly services for those living independently. Assistance may include daily living skills, transportation, medication monitoring, and community engagement.
Day Habilitation (LINKS) provides a structured, person-centered environment where individuals build independence through daily activities, skill development, and community involvement.
Intensive Residential Service Home (IRSH) offers 24-hour, community-based support in a small home setting for individuals who benefit from consistent supervision and behavioral health support.
Services are available across Trivium’s Iowa locations, with referrals currently being accepted.
